Houlton Jr./Sr. High School, Houlton
We utilize STAR for reading and math. In addition, we have bi-weekly team meetings and screen kids for grades and standards and develop protocols/action plans for each student. We like the STAR assessments, as we believe they are easier for progress monitoring because you can take them quickly and they give you instant data.

RTI Screening Tools for Behavior
Morison Memorial School and Hudson Elementary School, Corinth
Here is the SRSS (Student Risk Screening Scale) that we have used in RSU 64 since about 2008 when the middle school became involved in the PBIS implementation process. The screener was suggested to us by the team from the University of Maine that we were working with the implement RTI behavior procedures. We just personalized the heading.
Old Town Elementary School, Old Town
This screener comes from our 3-5 behavior team, but could be used at other levels. It is simple, but includes internal and external behaviors, which our team likes.
Wisdom Middle/High School, St. Agatha
Some of us are using Dojo, the student and parent have access too.
Lake Region Middle School, Naples
A free researched based behavior-rating tool is the Student Risk Screener Survey (Dr. Lane).

Our social workers found something on the web that is no longer even there – so no attribution available. We adjusted the format and some of the wording. We are using it district-wide to a small degree – it’s new this year.
The interventions sheet provides some suggestions as we look for the function of the behavior.
Same source, different formatting of the same information.
Teachers have found it effective in figuring out a next step for a behavior challenge.
